US Pipeline and Hazardous Materials Safety Administration (PHMSA) statistics on pipeline incidents, accidents and their many causes. This link shows the tables and charts of a 20-year historical view of accidents and incidents over all three types of pipelines.

US Pipeline and Hazardous Materials Safety Administration (PHMSA) compiles data on pipeline incidents, accidents and their many causes. Pipeline operators are required to report statistics to PHMSA including the primary causes of pipeline accidents and incidents. Using reported data from the most recent years, PHMSA provides the following National Failure Cause Report which summarizes the incidents and accidents attributed to all major cause categories.
